Input wanted for community health assessment in Phillips County

The Northeast Colorado Health Department is currently conducting a community health assessment that will direct its services focus for the next five years. This two-part assessment analyzes a number of community health indicators including risk factors, quality of life, mortality, morbidity, community assets, and social determinants of health and health inequity. 

In addition, information will be gathered on how effectively the public health system provides essential services. 

The data from this community health assessment will inform decision-making, the prioritization of health problems, and the development, implementation and evaluation of their next public health improvement plan.
